When a child is expected to be placed in your home and is coming from another foster home, unless he or she was abruptly removed, preplanning should take place as led by the child’s case manager.
Information shared with you should include:
• A pre-placement visit to your home
• An up-to-date Child Information Sheet (Appendix B) and Match Screening Summary
• The child’s most recent case plan
• All additional information and/or items that will help to minimize the trauma of the move for the child, including information regarding the child’s experience at his or her previous placement
• If appropriate, the opportunity for the child’s former foster parent(s) to share information about their parenting experience with the child
