When applying for Head Start for a child, the caregiver can present Form 2085 [sic, intranet link] Placement Authorization to verify the child’s eligibility status. The caseworker provides the caregiver with Form 2085 when the child is placed with the caregiver.
If a Head Start or Early Head Start program is not available in the local community or not appropriate for the child, the caregiver notifies the caseworker, who must document the information in the case file.
