- The information that the placing caseworker provided to the eligibility specialist.
- Data in the automated systems.
- The adoptive family.
- The eligibility requirements.
The eligibility specialist uses Form 2251d Adoption Assistance Eligibility Checklist for Non-DFPS PlacementsWord Document and the regional eligibility checklists to evaluate whether all of the eligibility requirements are met. The eligibility specialist places a copy of the appropriate checklist in the adoption assistance case record.
