Staff should complete Form 2138 (put hyperlink to Housing Plan here) with the youth at least 9
months prior, but no later than 6 months prior, to their 18th birthday to assist in securing the youth an
extended care placement at age 18 if they remain in care and to assist youth who are exiting foster
care at age 18 with independent living options.

For young adults in extended foster care, the caseworker should review the form with the young
adult 6 months prior to the young adult’s plan to leave extended foster care to assist the young
adult with independent living options.

Before completing the Housing Plan with the youth or young adult, provide and discuss with them
the Housing Readiness Handout [sic, broken link].
