A substitute care provider who prohibits a youth from living in their placement after the youth's
18th birthday shall notify the youth's caseworker:
• Not later than the 90th day before the youth's 18th birthday if the placement is a foster home.
• Six months before the youth's 18th birthday if the placement is a general residential operation.
• As soon as possible if the youth is placed in a foster home, or general residential operation less than six months before the youth's 18th birthday.
After receiving notice prohibiting the placement, the youth’s caseworker should verbally
communicate with the youth about the youth's living arrangements and document the substance
of the communication in their contact narrative.
The youth’s caseworker should work with the youth to update their Housing Plan (link to form)
and secure an alternate living or housing arrangement for them after they turn age 18.
